Easter Saturday - 4th April 22015. We will be open on Easter Saturday providing appointments from 8am -11am. If you are unwell and need to see a GP Please call during these times to book an appointment at the Surgery. At all other times please call the surgery and your call will be connected to our out of hours service.

New Patient Registration

consultation1The doctors welcome new patients who live within the postcode of MK6

New registrations are completed between 09:30 and 17:00 every day.  Please complete the registration form given to you at reception when you first visit the surgery.  To register you must provide one item of identification to show who you are and one to show where you live.  Each person over 16 must provide individual proof of address. 

If born in the UK we accept passport or UK birth certificate or drivers license, with a tenancy agreement or council tax letter dated within the last 3 months or bank statement dated within the last 3 months.  If not born in UK we accept passport with a tenancy agreement or council tax letter dated within the last 3 months or bank statement dated within the last 3 months.

All new patients will be asked to complete a health questionnaire and will need to have a 10 minute ‘new patient’ health check with the nurse .  This enables us to take a short history until your medical notes arrive from your previous GP.  Please inform the nurse if you are on repeat medication.
