New Patient Registration
The doctors welcome new patients who live within the postcode of MK6
New registrations are completed between 09:30 and 17:00 every day. Please complete the registration form given to you at reception when you first visit the surgery. To register you must provide one item of identification to show who you are and one to show where you live. Each person over 16 must provide individual proof of address.
If born in the UK we accept passport or UK birth certificate or driver’s license, with a tenancy agreement or council tax letter dated within the last 3 months or bank statement dated within the last 3 months. If not born in UK we accept passport with a tenancy agreement or council tax letter dated within the last 3 months or bank statement dated within the last 3 months.
All new patients will be asked to complete a health questionnaire and will need to have a 10 minute ‘new patient’ health check with the nurse. This enables us to take a short history until your medical notes arrive from your previous GP. Please inform the nurse if you are on repeat medication.
